Your Medical plan selection requires you to choose a doctor (provider) for each covered family member. It's important to choose one who's familiar with your medical history, can perform regular check ups, provide guidance and coordinate any care you may need.

Instructions:

• Click on the Search for a Provider link. This will take you to the provider directory for your plan.
• Choose the provider type, enter your zip code and click Continue.
• Enter any other search information and click Continue.
• Choose your state and click Continue.
• Once you locate a provider you want, copy his/her Provider ID Number.
• Enter that Provider ID in the space below.
